- HIPAA EarlyViewTM Privacy Tool
This is a software tool designed to help healthcare providers, plans and clearinghouses assess their current position in relation to the HIPAA privacy regulations and begin their compliance strategy. The tool was developed jointly by NCHICA (North Carolina Healthcare Information and Communications Alliance) and the Maryland Health Care Commission. A demonstration of the software is available on the NCHICA web site.
NCHICA has also reported that their HIPAA Privacy Work Group is "cranking out" documents (sample consent, authorization, notice, BA Contract, etc.) that can be referenced on line by the tool and imported into a document library so that a facility can build its own compliance plan.

Philadelphia: HCCA/AHA HIPAA Forum, 2001. CD includes sample policies and forms submitted by HCCA (Healthcare Compliance Association) and/or AHA (American Hospital Association) member or affiliated organizations.

- HIPAA Complaint Submission Form - Center for Medicare and Medicaid Services
This is the form to use to submit compaints about covered entities who are not compliant with the HIPAA electronic transaction and code set standards.
- Notice of Privacy Practices (PDF) - Department of Veterans Affairs
The Veterans Health Administration (VHA) has provided us with the VA Notice of Privacy Practices in which they state how health information and individual privacy rights are handled by the VHA.
