MA-HISPC Phase 2 - Consent Management Project
The Massachusetts Health Data Consortium has determined that a Consent Management
system based on interoperable business practices will substantially advance the
adoption of clinical health information exchange in Massachusetts. Based on the
findings of the Consortium’s MA-HISPC Project, a detailed implementation plan has
been developed for a consent management policy and technology project.
Through the eleven-month HISPC process, the Consortium was successful in
convening a large group of stakeholder representatives and active workgroups
to address the identification and prioritization of variations in Massachusetts’
business practices and privacy and security challenges to electronic health
information exchange (HIE) interoperability. After analyzing the most troublesome
barriers to interoperable HIE, we concluded that a standardized policy framework
for consent management that could be translated into a technical solution for
managing and implementing patient directives and permissions across enterprises,
is an essential element for the widespread and consistent adoption of electronic
clinical HIE.
The Consortium intends to proceed with a consent management project, including
the special issues around sensitive information (e.g. HIV/aids, mental health
data, etc.). The Consortium has identified a centralized policy structure as
one of the missing components needed to further the uptake and evolution of
successful interoperability in the Commonwealth. A Project Plan has been
prepared that begins with the development of policy and community requirements,
proceeds through the administrative specifications of a logical decision framework,
the formulation of technical specifications for a sematically interoperable
consent management system, and finally envisions the development and testing
of a pilot technology application solution.
The Consortium has demonstrated successful methods to manage such projects,
is the right organization to perform this project, and this is an ideal time to
commence. The Consortium benefits from the expertise, momentum, and enthusiasm
of the stakeholders, workgroups, and content management staff assembled for the
MA-HISPC project. These resources, along with the Consortium’s long-standing
position of excellence in the area of forums, education, communication, and
convening, will also be used to develop the education, communication and
dissemination programs and strategies critical for long-term success.
The Project will:
- Identify priority use cases for the application of HIE to clinical workflow
- Collect and inventory existing policy, procedure and practices around these use cases
- Analyze business practices and existing policies for commonality
- Compile the laws and regulations affecting these use cases
- Analyze variation between business practices and policies and the laws and regulations
- Develop a baseline of applicable policies (while acknowledging variations among communities)
- Communicate and educate all stakeholders (including consumers)
- Develop a decision matrix, administrative and technical specifications for consent management e-information exchange
- Build and test a Consent Management Application, through MA-SHARE, in an HIE environment broad enough to encompass
business practices, while being capable of handling variations.
Project Plan sets out 24-month initiative scheduled to begin summer 2007.
