Initiatives Promote Timely Access to Patient Information

Superior communication about patients and their medical data is a critical component of the quality equation, and panelists will discuss specific initiatives during the "Process Improvement/Reducing Medical Errors" session at the Consortium's 2004 HealthMart Conference, "Crossing the Quality Chasm: How do we get there from here?" on Friday, October 29, at the Westin Copley Place.

Moderator Paula Griswold, Executive Director of the Massachusetts Coalition for the Prevention of Medical Errors, will lead a discussion featuring Doris Hanna, RN, CPNP, ScD, a consultant to the Coalition, and, from Beth Israel Deaconess Medical Center, Larry Nathanson, MD, Director of Emergency Medical Informatics, Joseph Li, MD, Director of the Hospital Medicine Program, and David Feinbloom, MD, Academic Hospitalist.

Dr. Hanna is working with the Coalition on a Communicating Critical Test Results study through a grant from AHRQ through the Department of Public Health. She will discuss the study, involving about 40 hospitals participating in a collaborative model for improving communication of critical test results.

"We're at the point where we think we have some good ideas for improving performance," said Dr. Hanna. Working with hospital teams, the Coalition has refined a set of safe practices for ensuring timely and reliable communication of critical test results to the clinician who can take action. "Through the Collaborative, we have identified strategies to support the implementation of these safe practices, including strategies for overcoming the common barriers."

Dr. Nathanson will discuss Beth Israel's Emergency Department "dashboard," an innovation that leverages multiple patient databases to generate a Web page listing every bit of information Beth Israel has on an individual patient. The dashboard incorporates diagnoses, lab results, outpatient information, discharge studies -- a complete BI patient history. Emergency Department caregivers have the ability to interact with the information, order new tests and send alerts through the dashboard.

Dr. Li helped launch the hospitalist system at Beth Israel seven years ago to establish a means of communication between primary care providers and the hospital, and he will discuss the use of technology to make the system more efficient.

Fellow hospitalist Dr. Feinbloom is interested in clinical resource management and will explore how Beth Israel has leveraged the physician order entry system in a way that encourages people to use it without resentment but at the same time positively shapes behavior.

Beth Israel hospitalists are working on a project with low-molecular-weight Heparin, directed toward optimal dosing, with the aim of reducing adverse drug reactions. The also are involved in a hospitalwide program of triage and time-efficient delivery of critical lab results.
