Part I: "The Role of Clinical Data and Risk Adjustment in Public Reporting of Hospital Performance"

Workshop Description

MediQual and the Pennsylvania Health Care Cost Containment Council (PHC4) have had a longstanding relationship. Through PHC4, the Commonwealth of Pennsylvania has a singular exemplary record in public reporting. PHC4 has a continuous 17-year record of publicly reporting performance of the healthcare system in Pennsylvania. In May 2003, FACCT - Foundation for Accountability awarded the prestigious Ellwood Award that honors organizations and individuals whose accomplishments challenge the health care system to be accountable, accessible and consumer-centered to PHC4. PHC4 has been a vanguard in public reporting including repeated CABG performance reports at the provider level.

One underpinning of the role of PHC4 is a sound scientific and clinically defensible approach to risk adjustment strategies. In Pennsylvania, this has been supported through MediQual that provides both a statewide quality assured data collection methodology and the analytic support to produce the risk adjustment strategies used to support the public reporting effort. In this presentation, you will hear both the issues surrounding long term successful public reporting and also why clinical data such as laboratory findings and vital signs that are used to supplement the UB92 data set are critical to the risk adjustment modeling.

  • Public reporting remains an important topic in health care. There are growing recommendations for this to increase.
  • Normative comparisons in a world that respects privacy concerns and is HIPAA-compliant.

  • There are different criteria for data and risk adjustment models used for inter-hospital reporting than intra-hospital analysis.
  • Very different rankings can occur when using UB92 based risk adjustment than adjustment strategies based upon higher quality data.
  • The transparency of clinical risk adjustment strategies may foster enhanced acceptance of the results and subsequent change in responsive healthcare institutions.
